Posterior fossa infratentorial tumours represent about 50% of all intracranial tumours in children. They are usually intra-axial, extra-axial tumours being much rarer (see
brain tumour,
intra axial tumour,
extra axial brain tumour).
Contrary to the situation in adults in whom infratentorial intra-axial tumours are mainly secondary metastases, in children intra-axial infratentorial tumours are usually primary.
The most common tumours are cerebellar pilocytic astrocytomas, followed by medulloblastomas, brain stem gliomas and ependymomas.
Intrinsic pontine gliomas are infiltrative, and all are malignant regardless of their histology. Medulloblastomas (posterior fossa PNET) arise in the vermis for the vast majority; pilocytic astrocytomas have a hemispheric origin and are frequently cystic.
