Crazy paving pattern

combination of ground glass opacity and smoothly thickened interlobular septa having a patchy or geographic distribution, mimicking the appearance of paving stones (alveolar proteinosis (V:1), Fig. 1). This pattern suggests a diagnosis of alveolar proteinosis, but can be also be seen in other diseases, including pneumocystis carinii pneumonia, cytomegalovirus CMV pneumonia, pulmonary haemorrhage and bronchiolitis obliterans organizing pneumonia (BOOP).
