Musculoskeletal Imaging

Sinus tarsi syndrome

an injury to the structures in the tarsal sinus and tarsal canal, which include five ligaments (medial, intermediate and lateral roots of the inferior extensor retinaculum, the cervical ligament and the talocalcaneal interosseous ligament). Lateral foot pain, tenderness, and hindfoot instability are the characteristic manifestations. Among the causes of this syndrome are inversion injuries of the ankle.

On MR imaging soft tissue structures show poor definition, and oedema or fibrosis and abnormal fluid may be seen (Fig.1).

Sagittal fat-suppressed T2-weighted (a) and coronal inversion recovery (b) MR images of the ankle demonstrate increased signal intensity within the sinus tarsi. A small fluid collection also is observed posterior to the subtalar joint.
