Posttraumatic syringomyelia
Clinical history
This 44-year-old male was involved in a severe motor vehicle accident five years ago. He suffered a C7-T1 fracture with spinal cord compression, operated on acutely. The patient remained tetraplegic. Recently, the level of motor impairment had progressed upwards and an initial MRI, not shown here, demonstrated a large posttraumatic syrinx involving the entire cervical spinal cord.
The patient subsequently underwent shunting of the cyst.

Image 1: Sagittal T1WI. Persistent intramedullary cystic lesion at the level of the site of the old fracture (C7-T1). The posttraumatic hydromyelia extends cephalad to the C4-C5 disc level and is responsible for focal enlargement of the spinal cord.
