Vertebral artery dissection

Posttraumatic right cervical vertebral artery dissection, 1.5 T

This 40 years old male had minor neck trauma followed by severe vertigo.
The initial CT scan of the brain was normal. Dissection of the right cervical vertebral artery was diagnosed by MRI and subsequent MRA.

Fig.1 Transverse T1-weighted spin-echo image showing a narrow signal void residual channel (arrow) and the high signal of subintimal methemoglobin within the lumen of the right vertebral artery.

Fig.2 Sagittal T1-weighted spin-echo images clearly demonstrate that the dissection (subintimal hematoma) extends from C3 up to the C1 level (arrows).

Fig.3 Coronal targeted MIP reconstructions with angular differences of the cervical arteries from a 3D TOF acquisition data set showing the irregular wall morphology of the right vertebral artery associated with decreased intravascular signal intensity (arrows).
