Coiling of the internal carotid artery

Coiling and mild stenosis of the left internal carotid artery, 1.5 T.

Fig.1 The DSA image (left) with selective injection of the left common carotid artery shows coiling of the internal carotid artery and an associated mild stenosis at its origin (arrow). The targeted 3D TOF MR angiogram of the left internal carotid artery (middle) clearly demonstrates the presence of both the stenosis (arrow) and the coiling, as with this technique intravascular signal intensity is minimally influenced by changes in flow direction.
Conversely, the targeted 2D TOF MR angiogram (right) gives the erroneous impression of severe stenoses (arrows), because this technique is characterized by an inherent sensitivity to changes of flow direction, with in-plane flow leading to signal loss due to spin saturation.
