Metastasis of colon carcinoma
CLINICAL HISTORY:
This 70-year old woman underwent surgery 3 years ago for an adenocarcinoma of the sigmoid colon. Now she notes intermittent paresthesias of the right arm.
NEUROLOGICAL EXAMINATION is normal.
MRI:
a) TRANSAXIAL T2-WEIGHTED (2500/90/1) SPIN-ECHO SEQUENCE (IMAGES 1 AND 2): A mass lesion is seen (arrow) in the left side of the mesencephalon with pronounced hypointensity. Lesion is surrounded by edema.
b) TRANSAXIAL T1-WEIGHTED (600/20/1) SPIN-ECHO SEQUENCE (IMAGE 3): The lesion is slightly hypointense to brain.
c) TRANSAXIAL (IMAGES 4 AND 5) AND SAGITTAL (IMAGE 6) GADOLINIUM-ENHANCED SPIN-ECHO SEQUENCES: Ring-like enhancement of the lesion is noted.
RADIOLOGICAL DIAGNOSIS: Metastasis of colon carcinoma to the mesencephalon. The short T2 is typically described in metastasis of mucinous adenocarcinoma of the gastrointestinal tract.
No surgical confirmation is available.
