Meningioma with hyperostosis of the bone

CLINICAL HISTORY:
This 37-year old woman complains of 1 year of bifrontal headache. For 3 weeks she has noted atypical complaints of dizziness with tendency to fall.

CLINICAL NEUROLOGICAL EXAMINATION is normal.

CT:
a) NON-CONTRAST SCAN (IMAGE 1): A slightly hyperdense mass lesion is seen in the right frontal region. The lesion is broadly implanted on the convexity. The overlying bone (arrow) is considerably thickened.

b) CONTRAST-ENHANCED SCAN (IMAGE 2): Homogeneous enhancement of the tumor is noted.

MRI:
a) TRANSAXIAL T2-WEIGHTED (2500/90/1) SPIN-ECHO SEQUENCE (IMAGE 3): An extraaxial mass is seen in the right frontal region. The mass is slightly inhomogeneous, with overall isointense signal to grey matter, but with central hyperintensity. There is broad implantation on the convexity. The overlying bone (arrow) is considerably thickened, protruding centrally within the tumor.

b) CORONAL T1-WEIGHTED (600/15/1) SPIN-ECHO SEQUENCE (IMAGES 4 AND 5): The tumor is homogeneously isointense to grey matter. The overlying bone is thickened (small arrows).

c) CORONAL (IMAGE 6 AND 7) GADOLINIUM-ENHANCED T1-WEIGHTED SPIN-ECHO SEQUENCES: Intense contrast enhancement of the tumor is appreciated, with evidence of a dural tail. There is some contrast enhancement within the thickened bone. Note the dural tail (arrowheads).

ANGIOGRAPHY: (not illustrated) Selective injection of the external carotid artery lateral projection showed the typical radially oriented hypervascularisation of meningioma.

RADIOLOGICAL DIAGNOSIS is meningioma, based on the extraaxial location, with broad implantation on the convexity, the intense enhancement, the dural tail and the hyperostosis of the overlying bone.

SURGICAL FINDINGS:At surgery, the tumor was seen growing through the dura within the overlying bone. Total resection of the tumor together with the tickened surrounding dura was performed. The bone flap was partially removed.

ANATOMOPATHOLOGY points to fibrous meningioma. Bone invasion is confirmed.
