Chest x-ray and CT: Apical LUL mass along with significant mediastinal lymphadenopathy.
Transbronchial biopsy: SCC
Indication PET: Initial staging
Hypermetabolic foci on PET corresponding to lytic lesion in left transverse process C5 on CT consistent with bone metastasis
Teaching point:
Degenerativ facet joint disease may show very intense FDG uptake and may be hard to differentiate from malignancy when asymmetric (case xx). In this patient did the focus correspond to a osteolytic bone metastasis from NSCLC.
