Muscle strain

Overview

Sex: male
Age: 7 years

History
No significant past medical history.

Laboratory data
All laboratory values are within normal limits.

Physical findings
Palpable 3x8-cm anterior thigh mass of soft tissue consistency is noted. The mass appears to be within the substance of the rectus femoris muscle. The affected thigh is asymmetric as compared with the contralateral one. There is no functional deficit. Vascular and neurological examination of the affected limb is normal.

Case text
The patient is a 7 year-old male with mild pain in the anterior compartment of the thigh for 1 month. There is a non-specific history of an inciting traumatic event and the patient has not experienced sudden acute pain at any time.

1. What is the abnormality present on the ultrasound scans of the thigh?

A heterogeneous lesion within the muscle surrounded by a hypo-echoic ring.

2. What is your diagnosis?

Mass within the substance of the rectus femoris muscle.

3. What condition should be rule out?

Sarcoma of the soft tissue, which could be suggested because of the clinical presentation.

4. What is your next diagnostic step?

MR imaging.

T1-weighted SE images (600/15) showed muscle asymmetry, with the injured rectus femoris appearing slightly larger than the contralateral normal limb.

6. What is your diagnosis?

Rectus femoris muscle tear.

7. What condition should be ruled out?

Soft tissue sarcoma.

8. What is your next diagnostic step?

A biopsy of the lesion should be performed to exclude a soft tissue sarcoma.
